Hidradenitis Suppurativa

Hidradenitis Suppurativa (HS) is a chronic skin condition characterized by painful, inflamed lumps under the skin, often in areas where skin rubs together such as the armpits, groin, and under the breasts. Although it can be difficult to talk about and manage, early diagnosis and treatment are key to improving symptoms and quality of life.

What is Hidradenitis Suppurativa?

Hidradenitis suppurativa occurs when hair follicles become blocked and inflamed, leading to painful, swollen lumps or abscesses that may eventually break open and drain. These flare-ups can be recurrent, and in severe cases, they may lead to scarring or the formation of tunnels (sinus tracts) under the skin.

HS often starts after puberty and can persist for many years, with symptoms ranging from mild to severe. Although HS isn’t caused by poor hygiene, its appearance can lead to significant emotional distress and physical discomfort for those affected.

Symptoms of Hidradenitis Suppurativa

The symptoms of HS vary, but they commonly include:

  • Painful lumps under the skin: These lumps often appear in areas where skin rubs together, such as the armpits, groin, buttocks, inner thighs, or under the breasts.

  • Abscesses: These lumps can become inflamed and filled with pus, sometimes breaking open and draining.

  • Tunnels: In more severe cases, sinus tracts or tunnels may form under the skin between abscesses, which can be painful and difficult to treat.

  • Scarring: Repeated flare-ups and abscesses can lead to scarring and the formation of thick, hardened skin in the affected areas.

What Causes Hidradenitis Suppurativa?

The exact cause of HS is not fully understood, but certain factors may contribute to its development, including:

  • Genetics: A family history of HS can increase the likelihood of developing the condition.

  • Hormonal changes: Flare-ups may be triggered by hormonal changes, such as those occurring during puberty, menstruation, or menopause.

  • Obesity: Excess weight can increase friction between skin folds, leading to more frequent flare-ups.

  • Smoking: Smoking is a known risk factor and can worsen HS symptoms.

While hidradenitis suppurativa isn’t contagious or caused by infections, it can significantly affect a person’s quality of life due to the chronic nature of the condition.

Treatment Options

While there is no cure for hidradenitis suppurativa, there are several treatments that can help manage symptoms, prevent flare-ups, and improve quality of life. Treatment is tailored to the severity of the condition and the individual’s symptoms. Options include:

  • Topical and Oral Medications: These can include antibiotics, retinoids, or anti-inflammatory medications to help control flare-ups and reduce inflammation.

  • Biologic Therapies: Biologics, like those used in other inflammatory conditions such as psoriasis or Crohn’s disease, can target specific pathways in the immune system to reduce inflammation and prevent severe outbreaks.

  • Surgical Interventions: In severe cases, surgical procedures such as drainage of abscesses, removal of sinus tracts, or excision of affected skin may be necessary to prevent further complications and improve symptoms.

  • Lifestyle Changes: Weight loss, quitting smoking, and wearing loose-fitting clothing can help reduce friction and decrease the likelihood of flare-ups.
